How do I do this? Modding the grenade...

I was going to try and mod the Frag grenade into an Incendiary grenade making the explosion spout flames that attached to the player (or victim) much like the flamethrower.

How do I change the effects of the grenade? Do I need to use Guerilla?

no, you would just swap the explosion with flame_defoliant in HMT. not hard. (note: that flame wont last long)

actually u can change how long the flame lasts now with this one plugin..

but the flame will stick in one spot.

look at deimos's chain spawning mod, i was thinking of doing the same thing where, he did nades, i do fire, look at his mod and see if he has posted a tutorial on it yet, i think that that is how u do it
