Do I have a to make a sealed mesh and have all the faces with the fog material for it to work? How do I get my fog tag into the level?
What should I name this fog tag and where should I put it?

Fog is the same as water. It is in the tutorial, so I sugest ou start reading.

Yes, it explains I need a portal but, how do I apply it to a certain portal that I have made?
