Textures
Go into the material editor in 3ds max. Apply the bitmap to one of your materials. Select the faces you want to be grass, set their material id to the id of the grass, and viola!
Its pretty much the same as applying any other material. You do not have to use a ground texture, although you may want to reconsider as they tend to look better, and help the player navigate the various routes of the map.
